Forms & Fees

The Division of Corporations, Business and Professional Licensing makes available the following forms for use. In many cases, these forms meet just the minimum statutory and regulatory requirements. Although every effort is taken to ensure the forms comply with Alaska statutory and regulatory requirements, they should not be used in lieu of proper legal guidance from an attorney or other licensed professional.

Notice of Fee Changes: Effective February 29, 2012, the fees for the following filings changed to bring current fees* into alignment with Regulations (eff. 7/1/1980):

  • Notice of Change of Officials (for all entity types)
  • Registered Agent Resignation (for all entity types)
  • Non-profit Biennial Reports [3 AAC 16.050(c)]
  • Religious Corporations Biennial Reports [3AAC 16.070(b)]
  • LLC Certificate of Conversion [3 AAC 16.065(b)]
  • Foreign Cooperative Corporation Certificate of Authority [3 AAC 16.040(a), (b)]
